BIRTH CERTIFICATES

Montana Law allows a certified copy of a birth certificate to be issued only to the registrant (14 years or older), the registrant's spouse, children (with proof of relationship), parents, grandparents (with proof of relationship), a caretaker relative, guardian, an authorized representative or those who provide documentation showing it is needed for determination or protection of the individuals personal or property rights.
Uncertified (informational) copies shall be issued only for births that have occurred 30 years prior to the date of application.
NOTICE: STATE LAW PROVIDES PENALTIES FOR PERSONS WHO WILLFULLY AND KNOWINGLY USE 0R ATTEMPT TO USE A CERTIFICATE FOR ANY PURPOSE OF DECEPTION (MCA 50-15-114).
Each request for a certified or uncertified copy of a birth record must be in writing and contain the following information:

The registrant's full name, signature, and address

Copy of Valid ID

Date of Birth

The place (town or rural location and county) of birth

Mother's full maiden name (first, middle, last)

Father's full name
-
the applicant's relationship to the registrant

Reason for the request
-
Number of copies
CERTIFIED copy: $8.00
UNCERTIFIED copy: $2.50 (fees include postage)

We will no longer issue wallet-size certified copies, as of October 1st, 2006.
